Position Overview

Key Responsibilities
Conduct comprehensive assessments to determine the psychosocial needs of clients and their families.
Develop and implement tailored intervention plans that address individual circumstances and promote optimal well-being.
Provide counseling and support to clients and their families, helping them navigate healthcare decisions and cope with chronic illness or disabilities.
Coordinate with healthcare professionals to ensure integrated care and advocate for client needs within the healthcare system.
Facilitate access to community resources, including financial assistance, housing, employment, and mental health services.
Educate clients and their families about available community resources and support systems.
Maintain professional and accurate client documentation and prepare reports as required.
Participate in multidisciplinary team meetings to discuss and update client care plans.
Qualifications
Master’s degree in Social Work (MSW) required.
Licensed as a Clinical Social Worker (LCSW).
Experience in health care or a related field, with a strong emphasis on patient advocacy and support.
