Caremosphere is looking for detail-oriented and compassionate Care Activator to join our Circle of CareHearts. In this role, you’ll provide clinical oversight, complete client assessments, and ensure care plans are implemented with. Position Overview
As a Licensed Care Activator, you will be responsible for conducting client assessments, developing care plans, and ensuring quality care delivery. You’ll collaborate closely with CareHearts, monitor client progress, and provide essential clinical support to maintain the highest standards of care.
Key Responsibilities
Monitor and record patients’ vital signs, medical information, and changes in health to ensure effective management of their health status.
Administer prescribed medications and treatments in accordance with approved nursing protocols.
Assist with the development and revision of patient care plans under the supervision of registered nurses or physicians.
Provide basic patient care and treatments, such as dressing wounds, preparing and giving injections, and inserting catheters.
Educate patients and their families on health maintenance and disease prevention, following doctor’s instructions and established procedures.
Communicate with registered nurses and doctors to relay critical information regarding patient condition, treatment plans, and potential changes.
Ensure the cleanliness and sanitation of patient areas and assist with patient hygiene activities.
Participate in assessment data collection and implement interventions to improve patient comfort and wellness.
Qualifications
Diploma in practical nursing or an Associate Degree in Nursing.
Current LPN license in the state of practice.
Proven experience as an LPN or similar role.
Excellent understanding of nursing care procedures and methods.
Proficient in maintaining records and writing reports.
Outstanding organizational and multi-tasking skills.
Compassionate with good communication skills.
Excellent teamwork skills with an ability to interact efficiently with professionals from various disciplines.
