Position Overview
As a Care Navigator you will play a key role in being an liaison between clients, families, and healthcare providers to facilitate optimal care.
Key Responsibilities
Develop and maintain strong relationships with clients, understanding their ongoing needs and preferences.
Coordinate all aspects of client care, including scheduling, appointment setting, and communication between care teams.
Act as an advocate for clients, ensuring their care meets their physical, emotional, and social needs.
Provide regular updates to family members and involved healthcare providers.
Troubleshoot and resolve issues that arise during the care process.
Qualifications
Must have at least an associates degree in any healthcare related field.
Experience in care coordination or a similar role in healthcare.
Strong problem-solving skills and the ability to manage complex situations.
