Caremosphere is looking for dedicated and experienced Care Activator to join our Circle of CareHearts. In this role, you’ll provide clinical leadership, oversee client care plans, and ensure the highest standards of quality and safety. Position Overview
As a Care Activator, you will take the lead in client assessments, care planning, case reviews, and clinical oversight. Your expertise will be instrumental in ensuring compliance with best practices, mentoring CareHearts, and supporting seamless coordination of services to optimize client outcomes.
Key Responsibilities
Conduct comprehensive assessments to evaluate patient health status and determine appropriate nursing interventions.
Administer medications and treatments as prescribed by physicians while monitoring for efficacy and side effects.
Develop and implement detailed care plans in collaboration with patients, families, and other healthcare professionals.
Educate patients and their families on illness management, preventive measures, and post-treatment care.
Maintain accurate and up-to-date client records, ensuring confidentiality and compliance with relevant laws and regulations.
Supervise and mentor LPNs, CNAs, and other healthcare staff to ensure adherence to best practices and quality standards.
Respond promptly to medical emergencies, providing effective crisis intervention as needed.
Qualifications
Bachelor’s degree in Nursing (BSN) preferred or Associate Degree in Nursing (ADN) required.
Valid RN license in the state of practice.
Experience in a similar role, ideally within a community or home care setting.
Excellent clinical skills and innovation in nursing care.
Strong interpersonal, communication and leadership skills.
