Caremosphere is looking for dedicated and resourceful Care Navigators to join our Circle of CareHearts. In this role, you’ll be the trusted guide and advocate, ensuring clients receive the care and support they need. Position Overview
As a Care Navigator, you will be the bridge between clients, families, and care services, ensuring seamless access to the resources they need. Through compassionate support, strategic coordination, and problem-solving, you’ll help clients overcome barriers to care, empowering them to make informed decisions about their well-being.
Key Responsibilities
Develop and maintain strong relationships with clients, understanding their ongoing needs and preferences.
Coordinate all aspects of client care, including scheduling, appointment setting, and communication between care teams.
Act as an advocate for clients, ensuring their care meets their physical, emotional, and social needs.
Provide regular updates to family members and involved healthcare providers.
Troubleshoot and resolve issues that arise during the care process.
Qualifications
Must have at least an associates degree in any healthcare related field.
Experience in care coordination or a similar role in healthcare.
Strong problem-solving skills and the ability to manage complex situations.
Compassionate, client-first attitude with excellent communication skills.
Organized and detail-oriented.
Strong problem-solving abilities and attention to detail.
